For Existing Customers, looking to change their access to Products & Services
You must fill only the Amendment Form.
Original copies of all forms / agreements, must be submitted to any CBD Branch or your Relationship Manager for processing.
Amendment Form Filling Guidance:
Please only fill-out the relevant sections that are applicable to your applied services. In case of any doubt, kindly refer to your relationship manager.
Section 1:
Please input your company name as per Trade License
Section 2:
This refers to the accounts that you wanted to enable or disable with the chosen services
- Add - Fill this out if you wanted to add more services from your existing accounts
- Delete - Fill this out if you wanted to delete/deactivate services from your existing accounts
- Fill Section 2.1 - if you wanted to change your package from “view access” to "transaction access" access that or vice versa
Section 3:
- Number of Tokens for Corporate Users: Quantity of token to be deleted/Added for Corporate Users
- Delete Administrator Details: Details of Administrator that you want to deactivate
- Add Administrator Details: Details of Administrator that you want to add
- Other Token Amendments: Fill-out the option that is required to be performed on physical token
- Amend existing Administrator User Details: Fill-out only if you require to amend existing administrator
- Section 3.5 - Fill-out only if you require to add / delete any accounts on / from your existing access
Section 4:
- 4.1 Host to Host Details Change - If you want to change your current connection with the bank from semi host-to-host to host-to-host or vice versa
- 4.2 File Processing Details Change - if you want to change your current file processing from partial to file or vice versa
- 4.3 Change Address for Client Tool Implementation - if you want to change the machine where client tool is currently installed
Section 5 to Section 16:
Only fill-out if you require to add, amend, or discontinue the respective services
Section 17:
Only fill-out if you require to change your current set-up in collecting bank charges
